1. Age Requirement: Applicants must be at least 19 years old.

IF YOU HAVE CHILDREN OR SPOUSE, YOU CAN SIGN UP UNDER ONE APPLICATION.
2. Insurance Status: Applicants cannot have any other insurance, including Medicare or Medicaid.
3. Income Threshold: Your estimated income for 2022 must fall within the specified range.

To ensure accuracy and verification, all referral information must be correct, including your name, phone number, address, date of birth, and social security number.

We currently accept referrals and applications from the following states: Florida, Georgia, Kansas, Mississippi, North Carolina, South Carolina, Tennessee, and Texas. The income requirements for each household size in these states are as follows:

- Household of 1: $1,075 - $2,083 per month
- Household of 2: $1,451 - $2,333 per month
- Household of 3: $1,830 - $2,917 per month

If you reside in any of the following states: Alabama, Arizona, Arkansas, Delaware, Indiana, Iowa, Louisiana, Michigan, Missouri, Montana, Nebraska, New Hampshire, North Dakota, Ohio, Oklahoma, Oregon, South Dakota, Utah, Virginia, West Virginia, Wisconsin, or Wyoming, we also accept referrals from you. The income requirements for these states are:

- Household of 1: $1,657 - $2,083 per month
- Household of 2: $2,003 - $2,917 per month
- Household of 3: $2,525 - $3,917 per month
